Case Manager – Day Program

Department:              Student Support Services

Reports to:                Student Support Coordinator with departmental oversight by the Division Chair and Senior Director of Student Support and Engagement.

FLSA Status:             Exempt

Hours:                        Monday’s- Friday’s 8:30 am to 4:30 pm          



Purpose of position


Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet students’ and families comprehensive health needs through communication and available resources to promote school attendance and learning. Related activities to case management include complex condition management, promotion of wellness, and promoting transitions of care services.  The individual in this position is responsible for identifying students at risk for school failure associated with personal, familial, and/or school factors; assessing multiple health, education and social service needs; developing an integrated school-community service delivery plan; and advocating on behalf of students.  This individual may have specialized knowledge and expertise in a particular area such as immigration or adolescent homelessness.  The ideal candidate is creative, organized, culturally sensitive and has previous experience navigating social service agencies in the DC metro area. The case manager promotes student achievement and development by connecting students to available services and encouraging student participation in those services. The case manager works with students, parents/guardians, teachers, other service providers, school principals, and community-based resources to develop a healthy community both inside and outside of the school.

Student Support Coordinator with departmental oversight from Division Chair and Senior Director, Student Support and Engagement


Duties and Responsibilities


·       Maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.

·       Work collaboratively with Clinical Case Manger to locate and mobilize community resources for students.

·       Confers regularly with other stakeholders to assess needs of students.

·       Attend trainings and workshops to further understanding of psychosocial needs of the diverse populations served.

·       Promote and reinforce regular attendance with students, families, teachers, and administrators.

·       Participate as member of school-based teams to develop interventions for promoting students’ attendance and academic success.

·       Provide staff consultation and professional development on behavioral, social and environmental issues affecting student population.

·       Participate in weekly supervision with Director of Student Support Services.

·       Coordinate, organizes and maintains files for students assigned to caseload.

·       Consult and collaborate with clinical case manager, and school social worker to identify relevant and appropriate professional development trainings for staff and administrators.

·       Assess on both an individual- and group-level to determine student needs and how those needs fit into the school program.

·       Conduct home visits for students who are identified as truant.

·       Maintain communication and collaborative planning with coworkers with specific supportive roles (e.g. New Heights Coordinator, Homeless Liaison, Special Education Coordinator, etc.).

·       Attend meetings and professional development activities as required.

·       Conducts other duties as assigned.


Major Areas of Accountability


Adhere to laws related to DC Attendance Accountability Amendment Act, client confidentiality and mandated reporting.




Education and Training

Possession of a Bachelor’s degree in social work or related field such as Psychology or possess 2-3 years of experience in working in education, public health, human services and/other direct social service areas.  


Knowledge, Skills, and Abilities

Fluency in English; strong advocacy and leadership skills in working with individuals and groups (e.g. initiating individual and group discussion, listening, clarifying and facilitating interactions and sharing of ideas); strong writing skills; ability to counsel students, parents, staff and lay persons individually and in groups; ability to demonstrate effective liaison relationships with parents, schools and agencies; skill in communicating concepts and information accurately orally and in writing; ability and willingness to work with individuals from diverse backgrounds.
